This Privacy Policy describes how personal data and technical data are processed on the website of Zwierzak Veterinary Clinic, available at https://weterynarz-glogow.pl/ (the “Website”).
1. Data controller
The controller of personal data provided by the User through the Website is SIMES - SPÓŁKA Z OGRANICZONĄ ODPOWIEDZIALNOŚCIĄ, ul. Polna 10B, 67-200 Głogów, Tax ID (NIP) 6932058840 (the “Controller”).

2. Data that may be processed
The scope of data depends on how the Website is used. In particular, the following may be processed:
- data entered in the contact form: full name, email address, optionally a phone number, information about the animal and the message content;
- data voluntarily provided by email or during another form of contact with the Clinic;
- technical data related to use of the Website, such as IP address, date and time of the request, browser and device information, and server-log data;
- accessibility preferences stored locally in the User’s browser.
Please do not provide personal data in the online form that is not necessary to handle your enquiry.
3. Purposes and legal bases of processing
Data may be processed for the following purposes:
- handling enquiries, contacting the User and arranging appointments — where contact is aimed at entering into or performing a service agreement, the legal basis is Article 6(1)(b) GDPR; in other cases the basis may be Article 6(1)(f) GDPR, namely the Controller’s legitimate interest in correspondence and handling enquiries;
- ensuring Website security, preventing abuse and spam, and diagnosing errors — Article 6(1)(f) GDPR;
- establishing, pursuing or defending claims — Article 6(1)(f) GDPR;
- complying with legal obligations, where applicable — Article 6(1)(c) GDPR.
Selecting the checkbox next to the contact form confirms that the User has read this Privacy Policy. It is not marketing consent.
4. Is providing data mandatory?
Providing data is voluntary, but fields marked as required are necessary to send the form and receive a response. The phone number and information about the animal are optional unless they are needed for further contact in a particular case.
5. Contact form and abuse prevention
The contact form sends a message to the email address specified by the Controller. The Website theme does not store contact-form message content as separate WordPress database entries.
To reduce automated spam, the Website uses a form timing mechanism, a honeypot field and rate limiting. An IP address may be used to create a one-way cryptographic hash used solely to limit the number of messages. The hash is stored temporarily, for up to one hour.
6. Data retention
- correspondence data is retained for the time needed to handle the matter and then for a period justified by the need to document the contact, defend against claims or comply with legal obligations;
- if contact leads to use of the Clinic’s services, further data may be retained in accordance with applicable laws and rules governing records connected with the service provided;
- the IP-address hash used for contact-form rate limiting is retained for no longer than one hour;
- technical data in server logs is retained for the period resulting from the hosting provider’s configuration and policies and for as long as necessary to ensure the security of the Website.
7. Recipients of data
Data may be entrusted or disclosed to entities supporting the Controller in operating the Website and communications, in particular hosting, email, IT and maintenance providers, only to the extent necessary to provide those services. Data may also be disclosed to authorised public authorities where required by law.
8. Online booking via WetTermin and other external services
The “Book an appointment” / “Book an appointment online” button leads to the external WetTermin booking system available for the Clinic at https://www.wettermin.pl/lecznice/glogow/3811. After clicking, the User leaves the ZWIERZAK Website and moves to an external service.
The appointment-booking form is not hosted by the ZWIERZAK website. Data entered directly into WetTermin is not submitted through the WordPress contact form described in section 5. Booking is handled in the WetTermin/Klinika XP system in accordance with that system’s rules and relevant service/data-processing agreements.
According to information published by the provider, WetTermin is an integral part of the Klinika XP system. The provider publishes its own privacy and data-processing information. Before providing data in the external service, the User should also read the privacy information presented directly by that service.
The Website also contains links to other external services, including the Vetineo digital pet health record, Google Maps and the Clinic’s social-media profiles. These are links only; the current theme does not embed tracking scripts from those services.
9. Cookies and browser local storage
The Website does not run Google Analytics, Google Tag Manager, Meta Pixel, reCAPTCHA or marketing social-media scripts. It does not set its own analytics or advertising cookies.
WordPress and technical hosting components may use cookies strictly necessary for authentication, security and correct operation of the administration panel. These mechanisms are not used to profile visitors to the public Website.
The Website stores the User’s selected preferences locally in the browser: light or dark theme, text size, high-contrast mode and acknowledgement of the cookie notice. These data remain on the device, are not sent to the Clinic and can be removed in the browser settings.
If analytics, marketing tools or embedded third-party content requiring consent are introduced in the future, they will be blocked until the User makes a choice and this policy will be updated.
10. Rights of data subjects
Subject to the conditions laid down in the GDPR, a data subject may have the right to:
- access their data and receive a copy;
- rectify data;
- erase data;
- restrict processing;
- object to processing based on the Controller’s legitimate interest;
- data portability where provided for by the GDPR;
- withdraw consent at any time where a specific processing activity is based on consent, without affecting the lawfulness of processing carried out before withdrawal.

11. Right to lodge a complaint
If the User believes that their data is processed unlawfully, they have the right to lodge a complaint with the President of the Personal Data Protection Office (UODO) in Poland.
12. Automated decision-making
Within the functions provided by the current Website theme, data from the contact form is not used for automated decision-making or profiling.
13. Security
The Controller applies organisational and technical measures intended to reduce the risk of unauthorised access to data. The Website uses, among other measures, encrypted HTTPS connections, contact-form protections and mechanisms limiting automated abuse.
14. Changes to this Privacy Policy
This Policy may be updated if the Website’s functionality, services used or applicable laws change. The current version date is shown at the beginning of the document.
